First Visit
We look forward to meeting you! At your first visit we will help you get acquainted with our office and learn about your treatment options. You will be asked to fill out a health questionnaire, so please bring a list of medications you are currently taking. Your consultation will include a thorough head, neck and radiographic examination. We encourage questions from both the patient and parent.
During this visit we will provide a clinical diagnosis of the findings, outline your treatment plan, discuss anesthesia options, estimate fees and insurance co-pays. We will discuss all costs with you and give you estimated co-payment for your purposed treatment. We work closely with your dentist and/or orthodontist to take a team approach throughout your treatment with us.
Occasionally, simple procedures or emergency surgery can be performed the same day as the consultation. However, depending on the treatment plan, anesthesia options or medical history, it will most likely require a second appointment to provide your treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable.
- A list of medications you are presently taking.
- All medical and/or dental insurance cards, along with policy holder information. This will save time and allow us to obtain benefit information.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation and surgical visit.
X-rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
